Cancellation/No-Show Policy:

• Appointments should be cancelled at least 48hrs prior to scheduled time.

• We have a Card on File policy. In the case of a no call/no show a $100 fee is charged. 

• Self-pay rate: initial evaluation(60 min) $400

• Follow up visits(30 min) $250
